The Leicestershire Partnership NHS Trust has significantly increased the number of vulnerable patients across Leicestershire being digitally monitored at home, while also reducing the number of physical home visits needed, maximising local clinical resource and keeping patients out of hospital during COVID-19.
Patients who are being cared for by the trust’s community respiratory and heart failure teams, and those who have been discharged from hospital earlier than usual as a result of the pandemic, are being supported by Spirit Digital’s remote monitoring platform CliniTouch Vie.
Spirit Digital worked to make this latest technology available to patients nearly two months sooner than expected to help the NHS with its COVID-19 response, and has enhanced it to help medical teams quickly identify when a person exhibits health deterioration so they can intervene earlier. Remote monitoring also helps patients to know what to do next to help maintain their long-term health and wellbeing at home.
Put simply it is:
• Enabling patients to stay out of hospital and manage their condition at home;
• Connecting patients and nurses through technology to deliver care easily and safely;
• Empowering patients to learn more about their condition and nurses to be confident about the health status of their patient;
• Increasing hospital bed capacity for COVID-19 patients and maximising local clinical resource for community-based care.

CliniTouch Vie deployment started in Leicestershire in April 2020, with patients being added to the digital solution during the second week of April.
Spirit Digital and Leicestershire Partnership NHS Trust (LPT) jointly developed an on-boarding process, which included the patient set up, identifying each patient’s peripheral requirements, the delivery process, and a set up plan for patients.
Vulnerable patients in and around Leicester take regular vital signs readings and answer personalised health questions on a smartphone, tablet or computer. These readings are provided directly to specialist clinical staff who are able to prioritise patients based on their responses and their clinical observations. They can then remotely connect with patients to provide health and wellbeing advice, and intervene where and when more urgent care is needed.
Since April 2020, the trust was able to discharge a significant number of patients from the community hospital and monitor them remotely from the safety of their homes through this technology. As a result, the number of face to face home visits dropped by 35.1% per patient, making more efficient use of community-based clinical resources, during this challenging time.
Additionally, patients were empowered to self-manage their conditions at home, and were confident that they were safe.
Over six months, the projected savings would equate to over £80,000, reducing the overall costs associated with managing COPD and HF patients.
